Click on the order form link (HERE), print it, and fill it out completely. Please make sure to write both the show number and the air date. Then send it in along with a Cashiers Check, Money Order, or add your credit card information. If you are using a credit card (please) make sure to add your signature & date in the spot provided on the order form. Then mail it or fax it to the address on the bottom of the form.
